And this pride implies that for the daily difficult work, the essence of which is to help people and maintain the health of the inhabitants of our country, there should be decent pay. Because vocation is a vocation, but the more prestigious the work of a doctor will be, the more social guarantees she will have and the higher the salary, the more qualified specialists will graduate medical schools... But, unfortunately, at present, teachers' salaries leave much to be desired. Let's see if doctors should expect a salary increase in 2017 and what is their current salary level.

Current state and size of salaries for doctors

In 2014, there was a significant reduction in the number of health workers. This has led to the fact that, on average, each doctor works at 1.4 rates. In addition, medical institutions are increasingly changing the terms of remuneration under the contract in order to impose on doctors additional responsibilities as there are not enough doctors. At the same time, everything is legally correct - in defense of their decisions, the management of medical institutions refers to Art. 74 of the Labor Code. As a result, doctors do not have enough money, and they have to negotiate with the management in one way or another in order to work in 3 or 4 shifts, although legally they have the right to take a maximum of 3-4. Moreover, the management of many institutions began to conclude additional agreements with doctors labor contracts, according to which additional duties are assigned to the health worker as officials: if earlier these duties were paid in excess of the salary, then after the conclusion of additional. According to the agreement, doctors are obliged to work 12 hours instead of eight for the same, in fact, money.

A doctor's salary consists of several components:

l Incentive payments;

l Seniority allowances;

l Surcharges for harmful conditions labor.

There is a trick in this scheme, thanks to which the level of the average salary of a doctor, according to statistics, is growing. The fact is that the 35-36 thousand that a health worker receives on average, in fact, for the most part, consists of all these allowances. Moreover, these allowances are not a guaranteed part of the payment, that is, today they are - tomorrow they are not. For example, a couple of years ago, the scheme for paying the bonus for the harmfulness of work was revised: if earlier doctors belonged to category 3 (harmful working conditions), now even workers in the X-ray room belong to category 2 (permissible working conditions). And this brings with it not only a decrease in payments - but also the abolition of social benefits, including extra days which physicians who work 12 hours a day are physically necessary.

In turn, the payment of incentive and other bonuses mainly depends on the management of the medical institution, which "reshuffles" them in such a way as to deprive the employee of these bonuses. This happens everywhere and, in fact, for nothing - they can be deprived of bonuses for an error in the preparation of documents, they can be deprived of incentive payments for the fact that it takes too much time to see one patient (and sometimes, according to the system electronic queues, it should take 10-15 minutes to see one patient).

Should we expect a salary increase for doctors in 2017?

For a long time, the salaries of health care workers have been at unacceptable levels. However, this year we should count on the improvement of the situation - in the new year it is planned to spend more than a hundred billion rubles on salaries of medical workers. What is especially noteworthy, changes in payment will affect not only doctors, but also workers who do not have the appropriate medical education (for example, exercise therapy teachers). Also, the new salaries of nurses and nurses will be especially pleased - more than four hundred billion will be spent on salaries in the new year. It is too early to talk about specific figures, but one thing should be noted - changes are planned, and quite large-scale.

Who are civil servants and state employees

First you need to figure out who the state employees are. It is obvious that in everyday life this word "hides" all those people who receive salaries from the budget: officials, security officials, employees of state institutions, doctors, teachers, scientists. But to combine all state employees into one large category will not be entirely correct. Indeed, from the point of view of legislation, they are divided into different categories:

  • civil servants (persons working in government bodies, in government agencies);
  • workers budgetary sphere(teachers, scientists, doctors, kindergarten teachers, employees of cultural institutions and university teachers).

The difference lies not only in the name, but also in the approach to material support, subordination and social services... As usual, civil servants have it somewhat better. And their salaries and allowances are regulated by separate legislation. Government employees: doctors, teachers and accountants

According to rough estimates, about 33 million people work in the public sector in Russia. Of all citizens of working age (about 83 million people), the share of state employees is more than a third. Of these, about half are those who cannot be called civil servants, namely employees:

  • schools;
  • kindergartens;
  • secondary and higher educational institutions;
  • medical institutions;
  • libraries, museums and other cultural institutions;
  • scientific institutes.

They all get their wages from state budget RF: federal or local. Their working conditions are regulated by the Labor Code of the Russian Federation, as well as regulations developed by the department that manages the activities of the institution. For example, for doctors it is the Ministry of Health, and for cultural workers - the Ministry of Culture.

It is noteworthy that although all these legal acts (as opposed to law enforcement agencies) are open and public, the level of salaries of employees of these structures is officially difficult to understand. After all, the remuneration system presupposes not only a salary, but also a whole layer of various bonuses, subsidies and bonuses. Therefore, for example, the salary of a leading accountant in a budgetary organization may be the same, but different employees in different industries will receive completely different amounts on their hands.

However, when it comes to average salaries, all additional payments are usually taken into account when calculating them. So, according to Rosstat, the average salary in the country was 49,348 rubles (data for June 2020). Since statistics were sampled for all regions and cities, for small towns this indicator turned out to be overestimated: in practice, doctors and teachers in small municipalities can receive only 12,000-15,000 rubles a month. Salaries of state employees in 2020

The indexation of public sector salaries is part of Vladimir Putin's 2012 election program. The main document is Presidential Decree No. 597, sometimes called "Presidential Order 597: Roadmap 01.01.2018". Then, in May, immediately after taking office, the head of state issued 11 decrees that dealt with raising the salaries of civil servants and other state employees. Then the President promised to increase salaries by 2020, primarily for teachers and doctors.

Increase salaries primarily for those state employees who are subject to the Presidential Decrees:

The salary increases are phased. In the end:

  • for university teachers and doctors (medical workers with higher education), the salary level must be at least 200% of the average salary in the region;
  • for middle and junior medical staff and social workers, the salary level should not be lower than the average salary in the region.

Unfortunately, not all categories of state employees fell under the "May decrees". Obviously, there are a lot of those for whom the salary has not been increased. Therefore, officials promised not to bypass them and also to index their salaries. So, in accordance with the law "On federal budget for 2020 and for the planning period of 2020 and 2021 ", an increase in salaries of state employees is planned (see the table below for indexation rates).

Increase in salaries of state employees from 01.01.2019

It should be noted that some state employees will have to increase their salaries regardless of indexation. Indeed, from January 1, it is planned to increase the minimum wage (increase in the minimum wage), which will affect the salaries of junior and middle personnel with secondary education. From 01.01.2019, the minimum wage is 11 280 rubles. This salary increase will affect approximately 1.6 million people in the public sector.

Regional decisions on increasing public sector salaries

The order of the Government of the Russian Federation of December 28, 2012 No. 2599-r obliges the governments of the constituent entities of the Russian Federation to adopt orders on their regional "road maps", in which to prescribe specific figures for each year to increase salaries. So, the roadmap of the Sverdlovsk region to increase wages 2020 argued that the level of the average salary of social workers should be at least 100 percent of the average monthly salary in the Sverdlovsk region. And to increase salaries for public sector employees in 2020 in the Krasnoyarsk Territory, the Government allocated an additional 2.9 billion rubles from the federal treasury, so that regional allowances and the northern coefficient can be taken into account.

At the end of the year, regional authorities traditionally prepare appropriate decisions on increasing salaries for the next year. For example, in the Lipetsk region, the deputies decided to increase salaries in 2020 for state employees who do not fall under the May decrees by 10%.

Any problems in the Russian economy primarily affect workers in the "budgetary" sphere, most of whom are teachers and medical workers. Therefore, all fluctuations in the exchange rate of the national currency, a decrease in quotations for hydrocarbons or steps unpopular in the international arena in foreign policy, entailing tougher sanctions from other states - primarily affect the welfare of people who, due to the specifics of their work, cannot transfer it to "Commercial" channel.

The situation is aggravated by a vestige of the times of socialism - the declaration of the state on free universal education and medical care.

Why salaries are growing, but wealth is not growing

In Russia, in 2012, in the well-known "May" Decree of the President of the Russian Federation, an increase in wages was declared medical professionals for the period up to 2018. Wages should rise to 200% of the regional average.

Economic trends and high oil prices made it possible to build bright prospects and make loud statements. But at the end of 2014, the situation changed dramatically. A protracted economic downturn began. Along with it, inflation began to rise rapidly.

What they managed to realize by this time over the past two years "dissolved" in the general rise in prices for absolutely all consumer goods.

What is the cost of increasing the salary

The main direction that the state has chosen to raise wages is "optimization". Accrual in a new way - simply a fixed part of wages increases due to a reduction in the number of employees and an increase in the burden on those who remain in the industry.

At the same time, we are not talking about an increase in the total income of working medical workers, but only about an increase in salary. In turn, all existing bonuses are also “optimized”: bonuses “for harmfulness” are reduced, which can now be claimed by only those workers who work in workplaces with harmful factors, and not depending on the position held, bonuses for continuous length of service work.

Other payments are transferred to the management of hospitals, who independently decide the question of who should be paid additional remuneration, and who should not.

Prospects for the implementation of the program

The reform related to the increase in wages for medical workers was planned in two stages:

  • First step was aimed at optimizing and increasing the efficiency of all medical workers. It is almost complete.
  • Second phase- the working conditions of the remaining workers should be changed in such a way as to achieve the promised increase in wages up to 200% of the average wage in the region. With the fulfillment of this promise - there were problems.

The only fresh news is that no further reduction of medical workers is planned from January 1, 2017.

There is still a significant gap in the income levels of doctors with the same qualifications, depending on the region.

Average salary of doctors in Russia (by districts)

In average terms, it turns out that the salary of a doctor working in the Yamalo-Nenets Autonomous Okrug and in the North Caucasus is 66,247 rubles. In the same way, the average figures apply to employees of municipal and commercial medical institutions, the difference in salaries of which may differ by an order of magnitude.

In 2012, Vladimir Putin spoke about the need to raise the salaries of medical workers. This was stated in the so-called May decrees, which he also signed. The deadline for the implementation of one of the most famous May presidential decrees on a specific increase in salaries for public sector employees is January 1, 2018. By this time, the salaries of school teachers, nurses, cultural workers and some other categories of public sector employees should be equal to the average for the region.

Doctors, university professors and scientists should receive at least 200% of the average salary. The fact that the decrees have been successfully implemented is not specifically mentioned anywhere, but the figure - 35% for 2016 - appears as the level of income growth for medical workers. This increase in the salaries of health workers was due to the reduction in the number of health workers in hospitals and even the closure of some clinics, after which the budget was able to be distributed to fewer workers. 2017 is the last year for the implementation of the May decrees regarding the increase in the salaries of doctors. Today, we are already talking about changes in the payroll system in 2017. From January 1, 2017, the salary of medical workers in Russia will be charged in a new way. If previously the average salary of a medical worker was calculated only on the basis of statistics government agencies health care, since 2017, private clinics have been included in the calculation.

However, now, almost five years after the issuance of the decrees, despite the optimistic reports, it became clear that the salaries of public sector employees did not come close enough to the cherished indicators that their achievement in the remaining 11 months would seem realistic.

Even at the end of three quarters of 2015, the situation looked better than according to the latest data from Rosstat: over the three quarters of last year, the salaries of teachers and doctors in relation to the average decreased. Medical salaries decreased in nominal terms in 11 regions for nine months of 2016 - in Moscow, Yaroslavl regions, Nenets Autonomous Okrug, in Adygea and other subjects.

The salaries of teachers and social workers also do not come close to the targets of the decrees, according to Rosstat. The average salary in Russia for the nine months of 2016 amounted to 35,700 rubles. Social workers receive only 58.4% of this amount (20,900 rubles), university professors - 140% (50,000 rubles). For the same period last year, the ratios were about the same.

Closest to target 100% of school teachers' salaries. They amount to an average of 32,600 rubles, or 91.4% of the average salary in the region (on average in Russia). But the dynamics here is negative: for the same period in 2015 it was 96.5% with an average salary of teachers of 31,900 rubles.

In December 2016, Deputy Prime Minister Olga Golodets assured that the May decrees regarding salaries would be fully implemented. She also said that salaries, in particular for doctors, will increase in two stages: by 7.5% from January 1 of this year, at the second stage from October 1, they will reach 180% of the regional average, and by January 1, 2018 - 200%. Based on the achieved 137% for the three quarters of 2016, it turns out that the salaries of doctors for the year - from October 1, 2016 to October 1, 2017 - should show an increase of 63 percentage points to the average salary, which, according to the government's calculations, also will grow. Accordingly, the salaries of doctors should increase by about a third, after which they should add at least another 10% per quarter.

There is still a multiple difference in the size of doctors' salaries between the regions of Russia: from 23,200 rubles. in Ingushetia up to 109,000 rubles. in the Yamalo-Nenets and Chukotka Autonomous Districts. In Moscow, doctors earn an average of 81,000 rubles. per month, according to Rosstat. But if in absolute terms this salary is higher than in many other regions, then compared to the average in the capital it is lower (117.7%).

Regional dynamics are also different. If in the capital the salaries of doctors increased by about 10% (three quarters of 2016 compared to the same period in 2015), then in the Yamalo-Nenets Autonomous District, on the contrary, they decreased by 3%.

“Despite all the promises and measures on the part of the federal and regional authorities, over the past years, the task of increasing the salaries of medical workers has not been solved,” says Eduard Gavrilov, director of the Health Independent Monitoring Foundation. "It is obvious that the leaders of a number of regions are not coping with the implementation of the decrees."

A significant difference in salaries is also noted depending on the form of ownership of a particular hospital: it is more profitable to work in federal hospitals; in the first nine months of 2016, there was the largest increase in doctors' salaries (by 8%). In regional clinics, the salaries of doctors increased by approximately the level of inflation - about 5%, and in municipal clinics the growth was almost zero (negative, taking into account the rise in prices).

“The decrees will most likely not be fulfilled on time,” economist Yevgeny Gontmakher said in an interview with Gazeta.Ru. - Indeed, the gap is very large, such a miracle cannot happen even in two years, therefore, most likely not. The impracticability of the decrees was understood for a long time, one of the ways to solve the problem could be a postponement, but the president himself has not yet agreed to it. The day of reckoning will come sooner or later. The only question is: when? If this is the cutoff to January 1, 2018, that's one thing. But our colleagues from the government can say anything: 2018 will pass, and we will summarize ... The political situation in 2018 is changing - there will be a change of government and so on, so it is possible that the logic is to hold out until May. The most likely option would be a statement from the president about force majeure. The economic situation has deteriorated sharply compared to 2012, when the decrees were signed, so one can refer to insurmountable circumstances. "

The fact that the government is preparing to postpone the X-hour is indirectly evidenced by the adopted law on the budget of the Federal Mandatory Health Insurance Fund, which states that the federal subvention ensures the implementation of the presidential decree within the 180% ratio (for doctors) in 2017, and the indicator is 200 % is planned to be reached in 2018. This may mean that salaries will be raised during the 2018th election year, and not at its beginning, as it is implied in the decrees. Then, in 2019, it is planned to maintain the achieved level of the wage ratio.

Many contemporaries are interested in their future in advance. The doctors were offered information about the changes in their field. Of course, everyone knows about the predicament, about how much the data from the statistics of government sources and the actual salaries differ. Indeed, in the first case, everyone without exception became millionaires, but in practice they are trying to distribute their income, without the hope of resting abroad. And all this despite the fact that at work you have to plow for several, carrying out all the duties on your own. Moreover, this was introduced relatively recently. Against this background, it is worth considering whether there will be a salary increase for doctors in 2017.

Previously, narrow specialists practiced in their direction in such a way that their qualifications were directly related to that. At all times, only therapists who were authorized to refer patients to a specific referral specialist were involved in such responsibilities. To replace such a clear distribution of the responsibilities of physicians, related specialties were invented. Contemporaries prefer family doctors, whom they fully trust with their own health, their loved ones. But for this, the specialists themselves now need to master the basics of all areas of medicine, comprehending the carriage of knowledge in universities for a whole decade!

To justify so much time painstaking work, healthcare workers just need to see the salary increase for doctors in Russia in 2017 as a long-awaited change as early as next year. Otherwise, only grains of enthusiasts will remain from the crowds who want to learn and then heal people. The state is not interested in making the profession not prestigious, therefore the time for action is set for the next year.

What has been undertaken by the authority of the employees for medicine?

The head of state made efforts to solve the problematic situation of the creators of healthcare. Work is now underway to bolster wages. Growth there is possible through titanic efforts and the redistribution of financial budgetary resources. To do this, it was necessary to reduce cash receipts by ambulance and those on whom disease prevention depended. In the second case, they stopped transferring funds altogether, but in the first, a third of the previous amount remained.

Funds for indexation are definitely guaranteed. This is provided by the government in advance. Although, according to the experience of the previous year, it was possible to partially fulfill the obligations. But looking ahead from around the corner is not so comfortable either. It is not yet clear what will happen to housing and communal services, to prices in stores. Indeed, instead of the expected and financed 5.5%, the growth of the total monetary value turned out to be over 10%. So how significant the premiums will turn out to be is questionable.
