Nurse anesthetist profession description. Medical nurse - anesthetist, job description. Job responsibilities of an anesthetist nurse

Before the upcoming operation, the sequence of actions of the nurse-anesthetist is initially the same. In the ward or in the preoperative room, she premedicates the child to be operated on, and in the operating room prepares everything necessary for anesthesia.

First, she brings the anesthesia machine into working order, checks the oxygen pressure in the system and the presence of a spare oxygen cylinder, checks the operation of the suction, laryngoscope, compliance of endotracheal tubes and connectors, and controls the completeness of the medication set.

Then he proceeds to prepare his desktop, that is, he covers it with sterile linen and places syringes and needles on it, sterile material, balls moistened with alcohol, a set for catheterization of the great vessels and venesection, provides for a gastric tube, a urethral catheter, a working tonometer and a phonendoscope.

After reporting to the doctor about the readiness to work, the nurse-anesthetist collects in syringes all the drugs necessary to provide anesthesia, in accordance with the plan outlined by the anesthesiologist.

The child, delivered to the operating room, is laid by the anesthetist nurse on his back with the head extended, using a roller under his shoulder blades. For newborns and small children with imperfect thermoregulation, a special heated table or mattress with warm water should be used. At the same time, the nurse-anesthetist must very strictly monitor the heating temperature in order to prevent possible burns in this situation.

After placing the child, the nurse-anesthetist punctures his vein or connects the infusion system to a catheter located in the great vein. The introduction of the necessary drugs is then carried out through the tee.

With mask anesthesia, you need to choose a mask that is sized according to the child's age, so that it fits snugly to his face, and choose an appropriate size duct. An electric pump for sucking mucus from the oral cavity and preventing mucus aspiration should be constantly ready.

If endotracheal anesthesia is expected, then at the signal of the anesthesiologist, the anesthetist nurse slowly injects all the necessary medications into the patient's vein, and after the onset of relaxation gives the doctor an intubation set. By supporting the head in a posteriorly extended position, it helps the clinician to intubate and connect the endotracheal tube through the connector to the anesthesia machine.

Then she gives the doctor a sterile bandage, previously soaked in a solution of rivanol and wrung out, for tamponade of the oral cavity. This manipulation helps to seal the system and fix the endotracheal tube. In addition, narrow strips of plaster are used to fix the tube and connector, which the nurse-anesthetist must prepare in advance.

The nurse anesthetist has many responsibilities during surgery and anesthesia. She constantly monitors the operation of the anesthesia machine, periodically makes control measurements of the pulse, blood pressure, skin temperature and other physiological parameters. She performs all the appointments of the anesthesiologist and maintains an anesthesia card.

In it, she indicates the time of the onset of anesthesia and surgery and all stages of anesthesia and surgical intervention, physiological indicators obtained during control measurements, as well as the dose and time of administration of drugs.

She also informs the doctor about the rate of administration, the volume of transfused fluid and the amount of blood loss. Together with the doctor, she constantly monitors the child's condition, the color of the peripheral parts of his limbs, the depth and frequency of breathing, and the pupillary reaction.

The system of constant mutual monitoring helps to increase the safety of anesthesia and reduce the likelihood of complications. If any complication develops, the nurse immediately measures the child's blood pressure, calculates his pulse and heart rate, informs the doctor about everything and, at his command, takes the measures necessary to eliminate the complication that has arisen.

After the completion of the operation and the termination of anesthesia, the anesthetist nurse helps the doctor to sanitize the tracheobronchial tree, injects the necessary intravenous drugs, checks the level of arterial and central venous pressure, heart rate and pulse, reports the results to the doctor.

Continues until muscle tone and spontaneous breathing are fully restored. At this time, the nurse-anesthetist begins to calculate the total volume of infusion therapy, blood loss, and sums up the dose of relaxants.

After the restoration of adequate breathing, the child is transferred to the ward. The IV system is usually not shut off during this process, and the nurse anesthetist must be very careful to monitor the integrity of the system during transport of the child. The child is transported to the ward on a gurney by an anesthesiologist together with an anesthetist nurse.

In the ward, the child is placed on the bed in a lateral position to prevent the aspiration of vomit and saliva. His legs and arms are fixed with soft bandages. The child is in this position for 2 - 3 hours after the operation under the supervision of the ward duty personnel.

After the child is transferred to the ward, the anesthetist nurse completes the preparation of the anesthesia card, marking the end of the operation and anesthesia, and hands it over to the doctor. Then she turns off the anesthesia machine, processes the anesthesia equipment and instruments and dismantles her table, and when she finishes cleaning, replenishes the supply of medicines, infusion media and writes off the potent drugs, drugs and infusion media used during anesthesia and premedication.
